Case description:
A 58-year old woman had been treated for rheumatoid arthritis for 17 years. Occasional microscopic hematuria was presumed to be related to this treatment. A cystic lesion of the right kidney was incidentally found during a routine examination on ultrasound scan. Subsequently, CT and MRI were performed. They showed the 13-mm cortical cystoid formation with an enhancing fine wall and slight enhancing central portion. It was interpreted as a complicated cyst - Bosniak 2F. Watchful waiting with repeated imaging techniques was suggested. A year later, clear growth progression was observed with the biggest diameter being 20 mm. A cystoid tumor was suspected and partial nephrectomy with the safety margin was performed. A formalin-fixed specimen was sent for histologic evaluation. One picture of preoperative MRI is enclosed. Post-contrast T1 shows a small lesion in the right kidney with slight non-homogenous enhancement.
